Silent reflux, also known as Laryngopharyngeal Reflux (LPR), occurs when stomach acid flows back up into the throat, voice box, or airways. Unlike typical acid reflux, silent reflux doesn’t always cause heartburn, which is why many people don’t realise they have it.

Instead, it often leads to symptoms such as:

  • A persistent cough
  • A constant need to clear the throat
  • Hoarseness or a sore throat
  • A lump-like feeling in the throat
  • Excess throat mucus
  • Difficulty swallowing

Did you know? There are 2 types of reflux.

1. GERD
GERD (Gastro-Oesophageal Reflux Disease) happens when acid and stomach contents flow up into the oesophagus (the food pipe).
This causes heartburn, regurgitation, and sometimes a sour taste in the mouth.

Typical symptoms of GERD:

  • Burning pain in the chest after meals
  • Acid or food coming up into the mouth
  • Bloating or belching – worse when lying down.
2. Laryngopharyngeal Reflux (Gaseous/Silent Reflux)
LPR (Laryngopharyngeal Reflux) – also called Silent Reflux – is when stomach contents and gas travel even higher, reaching the throat, voice box, and airways.
Unlike GERD, most people with LPR don’t feel heartburn. Instead, they have irritating and annoying throat symptoms.
 
Common symptoms of LPR: 
  • Constant throat clearing
  • Feeling of a lump in the throat (=Globus)
  • Chronic cough or postnasal drip
  • Hoarse or tired voice/voice changes
  • Sore, dry, or irritated throat
  • Postnasal drip/mucus in the throat
What causes these throat symptoms of LPR?
When stomach contents reflux upward, tiny particles of pepsin can escape in gas form, even without liquid acid.
 
What is Pepsin?
Pepsin is a digestive enzyme made in the stomach that breaks down proteins.
When it reaches the throat or voice box, it becomes trapped in the tissues.
Even a small amount of acid or certain foods can reactivate pepsin, causing inflammation, swelling, and discomfort. In essence, pepsin damages the tissues of the throat (by its digestive/corrosive action), causing these symptoms.
